                                   ETENDER NOTICE
      FOR Hiring Services of Chartered Accountant Firm & Cost Accountant Firm For
         Concurrent Audit of Districts Health Society (DHS) for the F.Y. 2018-19
       Mission Director National Health Mission, Uttar Pradesh invites Expression of
Interest(EOI) from eligible Chartered Accountant firms & Cost Accountant Firms for conducting
monthly concurrent audit of the District Health Societies located at various districts of Uttar
Pradesh for the F.Y. 2018-19.






Eligibility Criteria : The firm must be three year old. Applying audit firm should submit
documentry proof for the above criteria.
        eTenders in two Bid System are invited from Chartered Accountant firms & Cost
Accountant Firms for for conducting monthly concurrent audit of the District Health Societies
located at various districts of Uttar Pradesh for the F.Y. 2018-19. The complete etender
documents may be obtained from the websitehttp://etender.up.nic.in OR http://www.upnrhm.gov.in.
The participating bidders should have valid Digital Signature Certificate (DSC) and have to register in the
portal http://etender.up.nic.in.     The tender should be submitted online in the portal
http://etender.up.nic.in. on or before 12.04.2018 up to 1:00 PM.
        The Bidder shall also submit the proof for payment like non refundable tender fee of Rs
500/ (Rupees Five Hundered) in the form of Crossed Demand Draft favoring "Districts Health
Society, respective district," payable at respective district headquarter.
        The Original Demand Drafts for Tender Fee must be dropped in tender box kept at CMO
Office of the respective districts on or before the last date and time of submission of the E-tender on
portal scan copy of DD attacted with proposal documents.
        Proposal documents without tender fee will be rejected.
